Southview Chiropractic & Wellness Centre (Southview) is committed to protecting the privacy and confidentiality of personal information. Our privacy practices are governed by the Personal Information Protection and Electronic Documents Act (PIPEDA), the Personal Health Information Protection Act (PHIPA), and applicable municipal privacy legislation and professional standards.

Southview collects, uses, discloses and retains personal information for the sole purpose of providing our patients with high quality health care services. Southview associates and employees are responsible for ensuring that your personal information is held in the strictest confidence. Southview's owner is responsible for the overall compliance to the requirements of all privacy legislation and Southview Privacy Practices.

Accessibility for Ontarians with Disabilities Act, 2005 (AODA)

Information for Patients

At Southview Chiropractic & Wellness Centre we are committed to meeting all requirements of the Accessibility for Ontarians Act, 2005 (AODA) with the goal of an accessible Ontario by 2025. We are committed to giving our patients with disabilities the same opportunity to access our services as those without disabilities. We provide the necessary and reasonable accommodation to prevent and minimize barriers to patients with disabilities so they can access, communicate and receive our services to the same extent as those without disabilities.

Clinic staff are provided with training including requirements under the AODA Act. Training includes: how to communicate with people with different types of disabilities; how to interact and support those with assistive devices, service animals or support people; and what to do if someone is having difficulty accessing our services. When communicating with a person with a disability, we do so in a way that takes into account their disability and offers reasonable and practical alternatives to support them. Personalized approaches are used to determine the best options to provide information and services in a timely and inclusive manner.

We support the use of assistive devices for patients while in our clinic and during care, and welcome people with disabilities who are accompanied by support persons. We also support those accompanied by service animals.

Southview Chiropractic & Wellness Centre accepts and respond to feedback/complaints regarding our services from patients, caregivers and/or the public by phone, in person or via email in our ongoing efforts to maximize the potential of all our patients through equal opportunity and participation.

Patient Rights and Responsibilities

Every patient has the RIGHT to...

  • expect to be treated with respect and courtesy
  • have ethnic, cultural, religious values and sexual orientation respected
  • expect that their personal health information will be kept confidential and private
  • participate in making decisions which affect their health care
  • have care and services explained clearly and simply in order to make informed decisions about their care

Every patient has the RESPONSIBILITY to...

  • treat Clinic staff with respect and courtesy
  • provide a safe environment for Clinic staff, free from harassment (physical and/or sexual) and abuse of any kind
  • participate in establishing their plan of care
  • notify Clinic staff of any concerns regarding their care, treatment or payments
  • attend scheduled visits on time OR notify the clinic at least 24 hours in advance of any cancellation or change
